We recognize that many may not understand where they should turn when a family member or friend is not ready or able to return home immediately after hospitalization from a major illness or surgery.
HAH Transitional Care (also known as Swing Bed) provides a viable alternative to a long-term care facility, such as a nursing home. Through our service, patients receive exceptional skilled care with an emphasis on maximizing health and rehabilitation potential. Our primary objective is to assist patients and their families in meeting their individual goals, whether it is to return home or transition to a new living environment. The scope of service available through HAH Transitional Care includes:
► Skilled Nursing Care for Wounds, IV Therapy, etc.
► Physical, Speech, and Occupational Therapy
► Nutrition Counseling
► Social Services and Activities
► Ancillary Services (lab, imaging, respiratory therapy)
► Discharge Planning
The Transitional Care Program at HAH provides individualized attention from our dedicated multi-disciplinary team of Case Managers, Physicians, Nurses, Physical, Occupational, and Speech Therapists, as well as an Activity Coordinator. We will meet with you and your family to discuss your plan of care. The use of a designated Transitional Care service such as ours meets a vital community health need and we invite you to call on us should you or a loved one have a need for this level of care.
